“Ask about a date” form

We use the data from the form (full name, phone number, optionally email and message) only to call you back and arrange a visit (Art. 6(1)(a) and (b) GDPR). We keep it for no longer than 12 months, after which it is deleted automatically. We do not share it with other companies or transfer it outside the European Economic Area.

Your rights

You have the right to access, rectify and erase your data, to restrict processing, to object and to withdraw your consent at any time. You may also lodge a complaint with the President of the Personal Data Protection Office (UODO).

Cookies and analytics

This website does not use tracking cookies or third-party analytics. The map (OpenStreetMap) loads from OpenStreetMap servers — only then does its provider receive your IP address.

Medical records

Health data is processed at the clinic in accordance with the Polish Act on Patient Rights — not through this website.
